Y-Lanyard Alert
This is important information for anybody who uses twin leg lanyards. Basically it appears a worker hooked twin leg lanyard to side D ring while not in use. During fall the distance to side D ring from anchor was shorter than to his back D ring - resulting in a tearing motion at lanyard intersection. Failure and death was a result.
